Local Presidents FSMTA Community Service Award

This award is given to a 9th grade student in recognition of his/her leadership, achievements, and service to the community in the field of music during the 7th, 8th, and 9th grades. Special emphasis is placed on service to the community which is the essence of the award.

Rules:
- Applicant must be finishing the 9th grade and must be a student of an FSMTA member teacher for at least six months.
- Applicant must be recommended by his/her FSMTA Member teacher and a music teacher and/or Principal of the student's school. The recommendation forms must be downloaded from the FSMTA website at this link and submitted electronically. Recommendations are due to the State Chairperson by December 31st.
- All applications are due to the State Chairperson by December 31st. If there is more than one entry from any county then the District Community Service Chairperson will be notified and all information sent to District Chairperson for a selection of the county winner. If there is more than one county within a District then there may be winners in each county. The winner is selected and information given to State Chairperson by February 1st. If the county winner lives in a different county that is not part of the Local MTA in which the Teacher is a member, it is the responsibility of the teacher to pay the $40 fee.
- Each FSMTA teacher will request a check for his or her applicant(s) from the local MTA treasurer to be mailed directly to the State Chairperson, Joanna Sobkowska (5305 Isabella Drive, Tallahassee, FL 32305) with the name of the applicant identified either on the check or a letter stating to which applicant the fee is being paid. The fee is due December 31st. If check is not received by the deadline the application will not be processed. The local MTA treasure makes the checks payable to FSMTA for each county applicant.
- The selection of the state winner is made by the FSMTA Community Service Committee. The state winner is presented an appropriate plaque, a certificate and a check for $500.00 at the End of the Year Awards’ Program for the winner’s high school. The winner will be announced no later than March 15th. Each county winner is presented a certificate and a plaque.
- Complete and submit Applicant Application Form including the Musical Community Service Activities, Musical Service Activities within School Activities, School Information, Short Biography, electronic photo and Applicant Statement electronically by the stated deadline of December 31st.
- A letter of recommendation from your Principal, Vice Principal, Music, Band, or Choral Teacher/Director must be submitted electronically by the stated deadline of December 31st. An interactive PDF form for this letter of recommendation is available on the FSMTA website at this link.
- A letter of recommendation from your FSMTA member teacher must be submitted electronically by the stated deadline of December 31st. An interactive PDF form for this letter of recommendation is available on the FSMTA website at this link.
- Along with this application, please submit a photo electronically to Chairperson of FSMTA Community Service Award. The winner’s photo will be used for publication on the FSMTA website and in the FSMTA newsletter.
